Artist's Beware Report Template

To start the month off, aside from the soon to come Positive Reviews post, here is the final completed reports template for the community, modified to wrap best around our ideas and your suggestions.
So from today forward, the mods strongly encourage you to follow this template when submitting a beware for approval.
This is the Artist's Beware Situation Report template. This template is here to assist you in getting your beware written out in a clear and fluid manner, get it approved in a much speedier fashion, and get your answers and advice to you faster than before! But before we begin, please take note of the following:
- Due to the potential lengthy nature of these forms/posts, please be prepared to insert an 'LJ-cut' into your post if you will be providing more than 3 links and/or the overall word count is over 1000.

- Screencaps (images taken of what is on your screen), are strongly suggested to be included into your post (they will be addressed in the template for the best place to include them). They are a big plus to your case, and can often answer any questions a community member might have for you.
- In a case of a multi-item beware regarding a single artist, please clearly list each article and their issues separately within each category of the post.
- This form is allowed to be changed/modified to fit your individual situation (i.e: trades rather than commissions). So if the field does not apply, you need not fill it out. That said, this template is for bewares, questions or advice posts need not follow this pattern.

WHO: List the aliases, business name or legal name of the person (Artist or Customer) this post is regarding. Which ever applies. Please note that if the artist or client does not use their legal full name in their business dealings, you must omit it from your report (this goes for screencaps as well).
WHERE: URL where they base their business out of. In case your beware is regarding something that occurred at an event (example: a con), you may list that too.
WHAT: Was it traditional art, digital art, sculpture, fursuit, jewelry, etc.? Were there multiple parts to this transaction? Here would be a good place to put any screencaps of conversation (emails/names that are appropriately blurred, PMs/Notes, IM logs, etc.). By conversation we mean the details hashed out between you and the Artist/Customer for said order(s).
WHEN: A rough time line of the situation from initial conversation, to 1st payment (or agreement of trade) to signs of trouble.
PROOF: State or place any screencaps of either Agreement (Notes, IMs, Shouts, Paypal screencaps with emails appropriately blurred, Money Order receipts, etc.). Note the proof of agreement includes trade as well.
EXPLAIN: Tell your side here. What happened, what went wrong, where did it go wrong, have you attempted to fix the issue(s), ask for updates, refunds/legal warnings, etc. Post any additional screencaps here that correspond with your explanation. Be as clear, concise, and to the point (no additional drama) as possible, without use of insults and slurs, please.
